The Workflow

Open HeyGen and have it ready before you start

Select the "Blank Form" template to start from scratch

Add the fields you need – for example, Name (short text), Email (email), and Phone Number (number)

Mark the Email field as required and customize field placeholders and validation rules

Enable an Autoresponder or connect to an email marketing tool to send a thank‑you email after submission

Go to the "Publish" tab, choose "Embed" and copy the generated HTML embed code

Paste the embed code into the HTML of your website where you want the form to appear

Optionally, connect the form to Google Sheets using Zapier
