Open Fillout.com and have it ready before you start

Sign up for a free Fillout account and log in to the dashboard

Click Create New Form, give it a name like "Consultation Booking", and open the drag‑and‑drop editor

Add Text fields for Name, Email, and Phone; add a Date Picker for the appointment date; add a Dropdown for time slots

Insert a Scheduling Page element, configure available dates and time slots, and enable automatic slot reservation

Set conditional logic so the Confirmation Message only appears after a time slot is selected

Open the Approvals tab, add an Approval Step, assign a manager’s email, and enable email notifications for pending and approved submissions

Upload your logo image, choose brand colors and fonts, and preview the design in real‑time
